Hiawatha Behavioral Health is seeking a Targeted Case Manager/Supports Coordinator to provide Case Management services to children with serious emotional disorders and their families. Facilitate linking, advocating, coordinating and monitoring of all services provided to the child and family. Serve as primary case worker to keep clinical record accurate, complete, and current. Complete assessments, and conduct treatment planning through family focused and person centered planning process.

Qualifications:
€ Bachelors Degree in Social Work with possession of LBSW licensure, or relevant degree in Human Services Field with possession of LBSW license.
€ One year experience working with children with Developmental Disabilities and Severe Emotional Disturbances preferred.
€ Proficient written and verbal communications skills
€ Proficient customer services skills
€ Proficient computer skills
€ Must be at least 18 years of age; possess a valid driver's license; and provide own transportation.
